Purpose of Position
Under the direction of the clinician, department supervisor, or designee, the Health Coach provides personalized support to patients, guiding them in making sustainable lifestyle changes to improve health outcomes. The Health Coach works as part of a medical care team, delivering high-quality clinical support and engaging patients in behavior modification, wellness education, and chronic disease management, in accordance with organizational policies and procedures.
Description
- Under the direct supervision of the clinician or department supervisor, assists in providing individualized health coaching and clinical support to patients.
- Assesses patients' current health status, barriers, and wellness goals, developing tailored health plans focused on nutrition, exercise, stress management, and overall well-being.
- Monitors patients' progress toward achieving health goals, providing ongoing coaching, support, and adjustments to health plans as necessary.
- Delivers health education and counseling on managing chronic conditions such as diabetes, hypertension, and obesity, in collaboration with clinicians.
- Greets and engages patients, building rapport and trust to encourage participation in their own healthcare journey.
- Inputs clinical information into the electronic health record (EHR) system accurately and timely, tracking patient outcomes and progress.
- Collaborates with the healthcare team to ensure patients receive coordinated, comprehensive care that aligns with clinical care plans and organizational goals.
- Supports and implements health coaching programs, workshops, and group education sessions to foster a culture of wellness.
- Maintains a high level of competence in patient communication and health promotion, staying up-to-date with evidence-based coaching methods and industry best practices.
- Assists clinicians by conducting health assessments, tracking patient metrics, and preparing patients for appointments or follow-up care.
- Coordinates with clinicians, department supervisors, and other team members to ensure smooth patient flow and effective care delivery.
- Participates in department meetings, trainings, and continuing education to promote professional growth and quality patient care.
- Ensures patient rights are maintained and abides by all confidentiality and HIPAA obligations.
- Takes initiative in staying informed of community resources and healthcare systems that support patient wellness and continuity of care.
- Triages walk-in patients per established protocols
- Verifies and implements clinician's orders for testing and/or procedures (EKG's, dressing changes, spirometry, Holter monitors, etc.)
- Provides effective and evidence-based patient and family teaching, using health promotion and health teaching methods in collaboration with the patient's values, beliefs, health practices, developmental level, learning needs, readiness and ability to learn, language preference, spirituality, culture and socioeconomic status
- Maintains patient rights and abides by all confidentiality obligations pursuant to CCHC and HIPAA
Qualifications
RN (X21B) - EDUCATION/EXPERIENCE/TRAINING:
- Ability to read, write and communicate in English
- Graduate of an accredited school of nursing
- Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts
- Certification as a Health Coach (e.g., National Board for Health & Wellness Coaching, ACE, NASM) required.
- Bachelor's degree in health coaching, public health, nutrition, exercise science, or a related field preferred.
- Basic Life Support Certification for Health Care Providers
- Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.)
- Minimum of two years' experience in a clinical setting preferred
LPN (P015) - EDUCATION/EXPERIENCE/TRAINING:
- Ability to read, write and communicate in English
- Graduate of an accredited Practical Nursing program
- Currently licensed as a Licensed Practical Nurse in the state of Massachusetts
- Certification as a Health Coach (e.g., National Board for Health & Wellness Coaching, ACE, NASM) required.
- Bachelor's degree in health coaching, public health, nutrition, exercise science, or a related field preferred.
- Basic Life Support Certification for Health Care Providers
- Strong interpersonal, customer service, time management skills
- Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.)
- A Minimum of two years' experience in a clinical setting preferred Cape Cod Hospital
